Breast Cancer treatment

About breast cancer

Breast cancer is the most common cancer among Indian women, and its outcomes depend heavily on how early it is found and how well the first surgery is done. Modern breast cancer treatment is highly individualised, combining surgery with chemotherapy, radiation, hormone therapy or targeted therapy as needed.

Dr. Ledwani has particular expertise in breast conservation and oncoplastic surgery, which uses plastic-surgical techniques to remove the tumour and reshape the breast in the same operation. For patients who need mastectomy, reconstruction options are discussed upfront.

Symptoms to watch for

  • A new lump in the breast or armpit
  • Change in the size, shape or contour of the breast
  • Dimpling, puckering or thickening of the breast skin
  • Nipple turning inward, discharge or crusting
  • Persistent pain in one area of the breast

Risk factors

  • Increasing age and family history of breast or ovarian cancer
  • BRCA1 / BRCA2 and other inherited gene mutations
  • Early first period, late menopause, late or no pregnancy
  • Obesity, alcohol use and physical inactivity
  • Long-term hormone replacement therapy

How it is diagnosed

  • Clinical breast examination
  • Mammography and breast ultrasound (sonomammography)
  • Core needle biopsy with receptor testing (ER, PR, HER2)
  • MRI of the breast in selected cases
  • Staging scans when indicated

Treatment options

  • Breast conservation surgery (lumpectomy)

Removal of the tumour with a clear margin while keeping the breast, followed by radiation. Survival is equal to mastectomy for suitable patients.

  • Oncoplastic surgery

Combining tumour removal with breast reshaping or reduction techniques so that the final result is natural and symmetrical.

  • Sentinel lymph node biopsy

Testing only the first draining lymph nodes, sparing most patients a full axillary clearance and reducing arm swelling.

  • Mastectomy with reconstruction

When conservation is not possible, skin-sparing or nipple-sparing mastectomy with immediate reconstruction can be offered.

  • Systemic therapy

Chemotherapy, hormone therapy or targeted therapy (such as anti-HER2 drugs) planned with the medical oncology team, sometimes given before surgery to shrink the tumour.

Frequently asked questions

Do all breast cancer patients need to lose the breast?

No. A large proportion of patients, especially those diagnosed early, can be treated with breast conservation surgery and keep their breast.

What is oncoplastic surgery?

It is a technique that combines cancer surgery with plastic-surgical reshaping in one operation, giving a better cosmetic result without compromising cancer control.

How soon can I return to normal activity?

Most patients are home within 1–3 days and back to routine activities in 2–3 weeks, depending on the extent of surgery.
