Oral Cancer treatment

About oral cancer

Oral cancer is one of the most common cancers in India, closely linked to tobacco chewing, smoking, areca nut (supari) and alcohol. It can start on the tongue, inner cheek (buccal mucosa), gums, lips, palate or floor of the mouth.

Caught early, oral cancer is highly curable with surgery alone. Dr. Ledwani specialises in organ-preserving head and neck surgery: removing the tumour with safe margins while protecting speech, chewing and facial appearance through careful reconstruction.

Symptoms to watch for

  • A mouth ulcer or sore that does not heal in 2–3 weeks
  • White or red patches (leukoplakia / erythroplakia) inside the mouth
  • A lump or thickening in the cheek, tongue or neck
  • Difficulty or pain while chewing, swallowing or moving the tongue
  • Loose teeth or dentures that suddenly stop fitting
  • Persistent bad breath, numbness or unexplained bleeding in the mouth

Risk factors

  • Tobacco in any form: gutkha, khaini, bidi, cigarettes
  • Areca nut / paan masala chewing
  • Heavy alcohol use, especially combined with tobacco
  • Poor oral hygiene and chronic irritation from sharp teeth
  • HPV infection and prolonged sun exposure (lip cancer)

How it is diagnosed

  • Clinical examination of the mouth and neck
  • Biopsy of the suspicious area for confirmation
  • CT or MRI of the face and neck to assess the extent of disease
  • PET-CT in selected cases to rule out spread
  • Dental evaluation before treatment

Treatment options

  • Wide local excision

Removal of the tumour with a margin of healthy tissue. For small, early cancers this may be the only treatment required.

  • Neck dissection

Removal of lymph nodes in the neck when there is a risk or evidence of spread, performed through a neat, low neck incision.

  • Mandible-preserving surgery

Marginal mandibulectomy and other techniques that avoid removing the full jaw bone wherever oncologically safe.

  • Reconstruction

Local or free-flap reconstruction to restore the tongue, cheek or jaw so that speech, swallowing and appearance are preserved.

  • Adjuvant therapy

Radiation or chemo-radiation after surgery when the final pathology shows high-risk features.

Frequently asked questions

Is oral cancer curable?

Yes. Stage I and II oral cancers have excellent cure rates with surgery alone. Even advanced cancers can be treated successfully with a combination of surgery and radiation.

Will I be able to speak and eat normally after surgery?

The goal of organ-preserving surgery is exactly that. Most patients return to a normal diet and clear speech after a short rehabilitation period, especially when reconstruction is planned from the start.

How long is the hospital stay?

Small excisions need 1–2 days. Larger procedures with neck dissection and reconstruction usually need 5–10 days.
